Our History
Chesapeake-Potomac Home Health Agency has been operating as a free-standing home health agency since 1995. The original home health license was actually owned by the St. Mary's Health Department. In 1994, the St. Mary's Health Department formed a partnership with the three local hospitals and opened the Chesapeake-Potomac Home Health Agency. The local hospitals and the Health Department formed the agency to make sure that home health services would always be available to patients residing in the tri-county area regardless of financial status of the patient.

The agency has noted a steady increase in the number of patient admissions and patient visits over the last decade. In FY2005, the agency admitted over 2,100 patients for home health services and performed about 33,000 visits. Many of our patients are frail and elderly. 75% of our patients are covered under the Medicare benefit.
