Description of Operation
Chesapeake-Potomac Home Health Care provides skilled, intermittent care to homebound patients under the orders of a Physician. Referrals for services are called to the office from hospitals, hospices, patient/family members, physicians, nursing homes, transitional care units and insurance case managers. Staff are assigned to initiate services based on specific patient needs and geographic area. Care is usually initiated within 24 to 48 hours of receiving the referral. A multi-disciplinary team treats the patient in the home setting and helps to improve the patient’s clinical and functional status. Patients are usually followed for approximately 4 to 6 weeks and discharged from services after making appropriate community referrals. Patients receiving long term, skilled care may be retained under services for a longer period of time. Our multi-disciplinary team communicates often and coordinates patient care so that quality services are provided in a cost efficient manner. |
